Care Coordinator

Essential Functions:

The Care Coordinator will work under the supervision of the Director of HIV/STD Testing & Prevention Programs. The Care Coordinator will provide non-medical case management for clients – conduct intakes, assessments, Individual Action Plans, and Linkage, Retention and Re-engagement to Care for people living with HIV. The Care Coordinator will provide appropriate active referrals and follow-up and is responsible for fostering a safe and trauma-informed environment. They will develop HIV treatment and retention plans with participants and follow up with in-person meetings, calls, and telehealth/virtual appointments. They will accompany participants to appointments and communicate with providers as necessary. The Care Coordinator will assist in the coordination of integrated care services. The Care Coordinator must also speak Spanish.

This position requires extensive knowledge of HIV/AIDS/STDs and available related services. This position requires the ability to work days, weekends, and nights including occasional late nights. Traveling within and outside the city of Chicago is also required and travel costs will be reimbursed.

 Job Description

Service Delivery Duties:

•    Meet with clients to assist them in filling out HIV treatment or medication coverage applications.

•    Assist clients in continuing HIV treatment and those looking to reengage in services with medical appointments and additional support.

•    Provide individual- and relationship-based HIV and STI counseling.

•    Provide education about Post-Exposure Prophylaxis (PEP), and other effective biomedical and prevention interventions; refer eligible, interested clients to primary care medical services to further discuss PEP and other preventive care with a provider.

•    Provide non-medical case management for patients returning to services, tracking client charts and ensuring retention to required follow-up medical services.

•    Implement and oversee health education strategies, interventions, and programs that address trauma, racial/ethnic inequities, and other social determinants of health.

•    Maintain highest level of client confidentiality; follow HIPAA regulations.

•    Enter progress notes within 24 hours of service provision.

•    Review participants files to ensure they are complete and ensure quality.

•    Report on a monthly basis on the number of sessions, referrals/services provided, and progress of each participant.

Program Development and Collaborative Duties: 

•    Assist in the continued implementation of HIV treatment services and appointment scheduling, while also ensuring that uninsured and under-insured clients receive education about coverage programs or linkage to insurance navigation.

•    Review and maintain up-to-date and accurate knowledge of emerging scientific research regarding biomedical HIV/STI treatment and prevention strategies, including PrEP, PEP and related interventions.

•    Help identify, develop, improve, and implement client education materials and other tools that encourage healthy decisions.

•    As needed and/or requested, assist the Director in developing the program and training new COH linkage to care staff.

•    Assist Center on Halsted staff during meetings and trainings which are required by funders, including the Illinois Department of Public Health and the Chicago Department of Public Health.

•    Coordinate with HIV Resource HUB staff regarding cross-referrals, Caseworthy, and other services for individuals living with HIV.

Data Duties:

•    Assist in updating tracking databases ensuring numbers are entered with a high level of accuracy.

•    Complete any agency, city, and state required forms for each client; maintain the highest level of client confidentiality; accurately collect and record all required and pertinent demographic information regarding each client served.

 Skills/Qualifications

•    Results – proven track record of exceeding goals and bottom-line orientation; evidence of the ability to consistently make good decision through a combination of analysis, wisdom, experience, and judgement; the ability to balance the delivery of programs against the realities of a budget; ability to solve problems, work independently, manage programs; ability to be creatively resourceful is essential.

•    Bachelor’s or Master’s degree in social work or health-related field preferred;  equivalent relevant experience will also be acceptable; experience working with individuals impacted by HIV/AIDS and STI’s is beneficial; prior experience working in a nonprofit environment is also beneficial.

•    Lesbian, gay, bisexual, transgender, and queer sensitivity; Knowledge and experience addressing LGBTQ needs.

•    Excellent oral and written communications skills; bilingual skills (Spanish/English) required.

•    A high degree of integrity in dealing with confidential information.

•    Ability to use MS Office programs, including MS Word, Excel and Outlook.

•    Effective problem solving and ability to analyze and use data for decision-making.

•    Effectively interact with people and develop positive relationships while being tactful, respectful, and direct in communication.

•    Ability to work flexible and extended hours including days, nights including late nights, and weekends required.

•    Knowledge of Chicago area public transportation required; ability to be reimbursed for transportation costs; comfortable traveling and working in all areas of Chicagoland.

•    Data entry experience required.
